  2. Opt-in now using our member form! Or, be on the lookout for a quarterly email with instructions on how to fill out our opt-in form once you qualify.
  3. Sit back & relax. Once opted-in, your gift card, which can be used toward millions of items store-wide at Amazon.com and certain affiliated websites, will be on its way every quarter you qualify.
